Instructions to fill the Registration Requisition Form

1. The first column should be kept free. It will be filled by Registration authority in the Office.
2. Please fill up your Name and Address in CAPITAL LETTERS carefully with Black ink pen in the second column of this form which will be taken for Registration Process.
3. Please sign on the specified place provided to customer (i.e. Signature of the Customer).
4. Thumb Impression should be from Left-hand only for Male and Right-hand only in case of Female
5. Thumb Impression should be from Black Ink Pad only.
6. Please Paste the Passport size Black & White Photograph Beside's the Thumb Impression.
